Physical Activity Readiness Questionnaire for
Everyone (PAR-Q+) is a questionnaire about your health history
that tells you whether it is safe to participate in physical activity
and exercise programs. Originally developed by the Canadian Society
for Exercise Physiology, the PAR-Q is a short questionnaire that
can help to identify possible symptoms of cardiovascular, pulmonary
and metabolic disease, as well as other conditions that might
be aggravated by exercise.
How does the PAR-Q+ Form work?
Before starting a new
exercise program, you are given the PAR-Q+ form.
You answer seven yes/no
questions. If you answer no to all questions, you're clear
to become more active in a progressive way and to follow
age appropriate guidelines.
If you answered yes (do
not panic!) to a question, you are prompted to answer a
few follow up questions that ask details about your health
condition.
If you answer no to all
follow up question you're cleared to become more active.
It's suggested you see a qualified exercise professional
for personalized tips.
If any conditions are identified, the form advises
the individual to seek medical approval prior to undertaking a
program of exercise, unless the trainer holds a specific qualification
related to the identified condition and feels fully confident
to deal with it. If no conditions are identified, the form suggests
a program of light to moderate exercise.
